Why it was raised
Queensland Rail's height-safety review after the overpass was set required a continuous static line the full length of the deck and a further 18 certified anchors. Not in the tender scope; instructed on site 22 July.
Crew 1 started this on 27 July under a verbal instruction from the package manager. It is the largest unapproved exposure on the book, approve it and it goes straight into the August claim.
